The mission of Evidence Sport and Spine is to be leaders in the delivery of safe, evidence-based, collaborative management of individuals with MSK pain through effective delivery of:
- Multidisciplinary clinical services
- Education and knowledge translation
- Clinical research.
Sunny Chhokar PT, MPhty (Musculo), MPT, BSc Kin, IMS Certified
Sunny graduated from the University of Saskatchewan with a Bachelor in Kinesiology in 2008, followed by a Masters in Physical Therapy in 2010. Following 3 years of private practice experience, Sunny moved to Brisbane Australia where he successfully completed his Masters in Musculoskeletal Physiotherapy degree in 2014 and has recently completed his training to perform IntraMuscular Stimulation/Dry-needling.
Sunny is very passionate about musculoskeletal physiotherapy and enjoys the challenge of treating a wide range of conditions. He is excited to work within the multi-disciplinary team in providing consultation for prolotherapy treatment and interventional radiology procedures for patients with acute or chronic conditions. Sunny is eager to continue with his post graduate education as well as working with the great team at Evidence.
Jeff Habberfield PT, BKin, MScPT, FCAMPT, cGIMS
Jeff graduated from the University of Calgary with a Bachelor of Kinesiology in 2005, followed by a Master of Science in Physical Therapy from the University of Alberta in 2008. Jeff has completed his Diploma in Advanced Manual and Manipulative Therapy (FCAMPT) and is a certified Gunn IMS practitioner.
Jeff is very grateful for the opportunity to learn from and work with the experienced Physiotherapists, Physicians and Nurse Practitioners at Evidence Sport and Spine. He has a special interest in spinal conditions and motor vehicle accidents. Jeff enjoys the opportunity to work with all disciplines to provide the best possible outcome for each patient.
Tanielle Garcia PT, MPT, BKin Hons
(currently on maternity leave)
Tanielle Garcia graduated from the University of Saskatchewan with a Masters of Physical Therapy in 2010. She graduated from the University of Regina in 2008 with her Bachelors in Kinesiology-Fitness and Lifestyle Honors Degree with a research component looking at the effects of neurofeedback on sport performance, as well as played university women’s hockey in the CIS (2003-2008). Tanielle is also a graduate of the Athol Murray College of Notre (2003).
Tanielle’s experience includes working in multi-disciplinary, private practice clinics where her case load includes a variety of musculoskeletal, orthopaedic conditions from children to the elderly population with a special interest in athletes. Tanielle has started to gain increased experience and training in the assessment and management of concussions and vestibular rehabilitation, and continues to increase her experience in manual therapy techniques and exercise prescription. Tanielle has also gained experience working as a volunteer physiotherapist with a non-profit organization called World Spine Care (WSC). She travels to their clinics in Botswana, Africa annually (started in 2012) providing physiotherapy treatment and education to people in underserved regions.
Renae Hunter - Pelvic Health Physiotherapist PT, BSc (KIN), MPT, Certified Postnatal Fitness Specialist
Renae graduated from the University of Calgary with a Bachelor of Science in Kinesiology in 2007 followed by a Masters in Physical Therapy from the University of Western Ontario in 2009. Renae is a certified Pelvic Floor Physiotherapist treating a variety of pelvic health disorders including prolapse, dyspareunia, urinary and fecal incontinence and pelvic pain. She is also certified in ROST Therapy for the treatment of pregnancy-related pelvic girdle pain. In addition, she has completed training in acupuncture through the Acupuncture Foundation of Canada Institute and has completed her Level 2 certification in Manual Therapy through the Canadian Physiotherapy Association Orthopaedic Division. She has a passion for patient-centred care and works hard to help patients achieve their maximum potential and overall well-being. Renae grew up as a competitive soccer player competing as a Dino for the University of Calgary for 4 years and continues to enjoy living an active lifestyle. Outside of the clinic, Renae is a busy mom of 2 and enjoys spending time with her family and exploring the outdoors.
Ashley Duguid - Physiotherapist PT, BKin, MScPT
Ashley graduated from the University of Calgary with a Bachelor of Kinesiology in 2009 followed by a Masters of Science in Physical Therapy from the University of Alberta in 2011. Ashley has since certified as a Pelvic Floor Physiotherapist and treats a variety of pelvic health disorders including pelvic pain, urinary and fecal incontinence, pelvic organ prolapse and dyspareunia. She has training in pre and post natal exercise prescription and pelvic pain management. Ashley also has a passion for orthopedic physiotherapy, treating a wide variety of muscloskeletal conditions including post motor vehicle accident, post surgical and sport related injuries.She is certified in functional dry needling, completing levels I and II with KinetaCore Dry Needling. Ashley has completed her Level 2 certification in Manual Therapy through the Canadian Physiotherapy Association Orthopaedic Division.Ashley is excited to be part of such a great interdisciplinary team at Evidence Sport and Spine. She looks forward to treating as both a pelvic floor and musculoskelatal physiotherapist.
Loring Derry - Physiotherapist PT, BA (Hons), MScPT, Functional Dry Needling
Loring completed her Masters of Science in Physical Therapy at the University of Alberta in 2014, following her BA (Honours) at the University of Victoria in 2010. She has achieved her Level III certification in manual therapy through the Canadian Physiotherapy Association’s Orthopedic Division. She is also certified in functional dry needling.
Loring is a former National-level Rhythmic Gymnast and is proud to have competed at the Canada Games and at events in Bulgaria and the USA. Her experience as a former athlete has fueled her passion for helping others improve their health through movement. Loring is passionate about treating chronic and complex spinal conditions. She is especially interested in treating chronic injuries related to dance and the performing arts. Loring is excited to work amongst the team of practitioners at Evidence Sport and Spine.
Dr. Jonathan Chow - Musculoskeletal/Family Medicine BSc, DC, MD, CCFP
Dr Chow originally did his training as a chiropractor and had a sports therapy practice in Calgary. He branched into medicine and retrained completing his medical school at the University of Alberta and residency at the University of Calgary. His current clinical practice consists of musculoskeletal injuries and back pain. He uses a variety of treatments including ultrasound guided prolotherapy, platelet rich plasma injections, and trigger point injections. He spends his time between both Evidence Sport and Spine North and South locations. Outside of work he enjoys spending time with his wife and children, mountain sports, and martial arts.
Lloyd Tapper - Nurse Practitioner PhD, RN, NP, ACCN Regenerative Medicine
PhD prepared nurse practitioner Lloyd Tapper works extensively with clients suffering from acute and chronic pain associated with muscular and skeletal injuries. In addition to the use of pharmacological and non pharmacological pain management strategies, Lloyd provides ultrasound guided regenerative medicine services including prolotherapy and platelet rich plasma therapy. His partnership and experience working with multidisciplinary health care team members including physiotherapists, chiropractors and personal fitness trainers ensures that each client receives comprehensive care.
Lloyd's academic preparation in the use of regenerative medicine includes completion of a clinical internship at the Advance Spinal Care Centre located in Calgary, Alberta, and the completion of additional training through the Department of Rheumatology at McMaster University. He has also completed training in the use of regenerative medicine and muscular-skeletal ultrasound under the supervision of Dr. Thomas B. Clark.
In addition to his clinical expertise and warm personality, Lloyd provides guest lectures focused on pain management, and the assessment, diagnosis and management of orthopaedic injuries. Further, he is licensed to practice in both Canada and the US.
Shalyn Huehn - Office Administrator/Manager
Shalyn is excited to join the Evidence Sport and Spine South team and is looking forward to bringing her many years of experience to the Office Manager role. She graduated from Bow Valley College with a Medical Office Assistant Certificate in 2013 and since, has been involved in many clinic aspects. Shalyn also has a background in Animal Health as she has a passion for animals.
Shalyn can empathize with Motor Vehicle Accident patients, as she is a survivor of one herself, in 2011. Since then, she has been a supporter for Stars Air Ambulance and has attended multiple fundraising events. She was in the 2012 calendar campaign, 2013 commercial campaign and also the 2015 Stars documentary; The Golden Hour. She looks forward to bringing her knowledge and her fun personality to the team and is enthusiastic about ensuring each patient receives the most optimal care.
Rachel Williston - Office Administrator/Manager
Rachel is a true believer in being an advocate for every patient she comes in to contact with. She started to pursue a career in health services in Lethbridge where she studied Psychology. She then moved on to become a certified EMR and hospital unit clerk. Most recently she can be found in her spare time pursuing Nursing (RN) through distance learning at Thompson Rivers University. When not studying Rachel can be found spending her spare time at the gym, spin class, exploring Alberta or camping with her partner and pitbull. Rachel is excited to be a part of the Evidence South team while covering a locum position.
Karla Selmer - Office Administrator
In February 2018, Karla graduated from Bow Valley College with a Medical Office Assistant Certificate. Prior to attending Bow Valley College, she spent just over 12 years working as an Educational Assistant in the Foothills School Division. Karla is passionate about helping others and believes in building positive, meaningful relationships. These values that she holds dear are what led her to a new career in healthcare. Karla is looking forward to being able to assist patients on their health care journey and is very excited to join the team at Evidence Sport & Spine. In her spare time, Karla enjoys baking, walking, yoga, golf, camping and hiking with her family.
Megan Tochor - Office Administrator
Megan graduated in April 2018 from UBC Okanagan with a Human Kinetics degree. Since then she has also completed her 200 hour yoga teacher training and is certified in Hatha yoga taught in a therapeutic style. She has a passion for movement and wellness and is excited to explore further education in the future. In her spare time you can find her teaching and practicing yoga, snowboarding, spending time hiking around in the mountains, or surfing off of Vancouver island. Megan is thrilled to be a part of the Momentum team and to help others on their path to recovery and wellness.
Lona Samadi - Office Administrator
Lona is currently studying finance at the University of Calgary. She has been working in administration for about a year now, but has many years of customer service experience. Lona is very goal oriented and works very well with others. She has a passion for learning, so that she can use her knowledge and experience to help others. Lona has lived in Calgary for almost her whole life. In her spare time, she likes to play her favourite sport, basketball. Lona has played basketball for 6 years until recently, when she had to give it up so that she could focus on her studies. She also enjoys going hiking and spending time with friends and family.
